Creating Access to Resources & Expanded Services (CARES)

The CARES (Creating Access to Resources & Expanded Services) Program, a collaboration between NEWAHEC, Milwaukee AHEC, and Ground Game Health, offers college students a unique community health field placement opportunity focused on addressing social determinants of health and barriers to healthcare access.

Participants in the CARES Program engage in virtual patient assessments to identify social determinants of health and other factors hindering patients from accessing necessary healthcare services. Students commit to a minimum of 40 hours, which includes completing virtual patient assessments.

Eligibility/Requirements

  • 40 hours

  • Completed virtual assessments

  • Thorough Eletronic Medical Record (EMR) patient documentation

Utilizing Community Resources

Students are equipped with knowledge of available community resources and support services to help patients address their healthcare needs effectively.

Motivational Interviewing

Training in motivational interviewing techniques enables students to engage with patients in a supportive and non-judgmental manner, empowering them to make positive health behavior changes.

Virtual Patient Assessments

Participants learn how to conduct thorough patient assessments remotely, ensuring comprehensive care planning and coordination.

Entering Assessment Information

Students receive training on accurately documenting assessment findings in health insurance databases, facilitating continuity of care and coordination among healthcare providers.

Upon completion of all training requirements, participants receive a certificate of completion, recognizing their dedication to enhancing community health and addressing healthcare disparities.
