How To Apply
The Health Careers Summer Camp uses an online application that must be completed once it is begun. Since you may not “save as you go,” you are encouraged to first work through the application offline using the PDF version of the application. If you complete your essay using a word processing program prior to completing the online application, you may simply cut and paste your essay into the online application form. There are links below to both the online application and the PDF version of the application.
To complete your application, you will need to have two teachers complete an online teacher recommendation form. There are links below to the “Request for Teacher Recommendation”, which you may print off and give to the teachers from whom you are requesting a recommendation. This request form includes the URL address to direct your teachers to the online recommendation form they need to complete for your application.
All students selected to attend the Health Careers Camp will be required to have current immunizations! Health facilities require students be current on immunizations and have either a two step TB skin test or the Quantiferon blood draw test completed within the last year – no earlier (Test result must be negative). A copy of your TB skin test must be included with the rest of the camp paperwork mailed to Northeastern WI AHEC. All students must also accurately complete the Medical Form with current immunization information!
The average TB skin test cost ranges from $10.00 to $20.00. The student must return within 48-72 hours after each of the tests have been administered, for a reading of the TB test results (a total of 4 visits). Please contact your local health department or Doctor’s office for details.
