The Health Careers Summer Camp is now using an online application that must be completed once it is begun. Since you may not "save as you go," you are encouraged to first work through the application offline using the PDF version of the application. If you complete your essay using a word processing program prior to completing the online application, you may simply cut and paste your essay into the online application form. There are links below to both the online application and the PDF version of the application. You will also need to print out the two teacher recommendation forms necessary for a complete application. The teacher recommendation forms need to be signed by you the student and your parent/guardian then given to the teacher to fill out and send back to the camp director. • Student Personal Statement- Essay (Online)
• Math or Science Teacher Reference (Hard copy sent to camp director)
• English or Fine Arts Teacher Reference (Hard copy sent to camp director)
